PKA dirty repo cleared: committed vault_mcp.py hybrid-recall code + regenerated served dashboards + graphify outputs, removed redundant .bak leftover, added *.bak gitignore; pushed to origin/master, tree clean.

Verified pushed to origin/master: 548411c9 (hybrid FTS+vector recall upgrade in vault_mcp.py + ignore stray .bak), 8537aca4 (regenerated served dashboards), 33d1f801 (graphify refresh). vault_mcp.py now carries the hybrid code (29 hybrid/vec/embed refs); *.bak gitignored. Dirty-repo resolved. Shadow-review fail = can't inspect a clean-tree state (blindness).
