MC-4535

Root cause from controller repair on 2026-06-01: ticket runtimes for MC-4506 and MC-4460 closed almost immediately and moved tickets to in_review because the parser/harvester treated echoed prompt/control text containing `DONE:/REVIEW:/QUESTION:` as a real worker signal. Acceptance: parser ignores echoed instructions/prompt/TUI chrome; only explicit worker-authored terminal signal or structured JSON from mc-coord transitions ticket state; add regression tests; requeue MC-4506/MC-4460 after fix. Expected check-in: 30m.
