Found it. Check C `check_execution_failures` made this ticket. The 07:28 row = intentional shepherd dedup cancel. Recovery-suppression only checks newer task_run success — but canonical rerun was worker-owned (MC-4402), not a task_run, so not suppressed → false-alarm ticket. Real fix: suppress when failure output = intentional shepherd cancellation. Scoped, prevents recurrence.
Dev-loop first (hook requirement), then check test conventions.
